Important — please read. This is not insurance.

This is a discount program, not insurance, not intended to replace insurance, and does not meet the minimum creditable coverage requirements under the Affordable Care Act. It provides discounts at certain health care providers for medical services. It does not make payments directly to the providers of medical services. You are obligated to pay for all health care services, but will receive a discount from those health care providers who have contracted with the discount plan organization. The range of discounts will vary depending on the type of provider and the service received. This program contains a 30-day cancellation period.

Discount Plan Organization: New Benefits, Ltd., Attn: Compliance Department, PO Box 803475, Dallas, TX 75380-3475, 800-800-7616. Website to obtain participating providers: MyBenefitsWork.com.

The discount program provides access to the Aetna Dental Access® network. This network is administered by Aetna Life Insurance Company (ALIC). Neither ALIC nor any of its affiliates offers or administers the discount program. Neither ALIC nor any of its affiliates is an affiliate, agent, representative or employee of the discount program. Dental providers are independent contractors and not employees or agents of ALIC or its affiliates. ALIC does not provide dental care or treatment and is not responsible for outcomes.

Savings are based on the provider's normal fees. Actual savings will vary by location and the services or products purchased. This discount program is a referral plan and makes no warranties concerning the quality of care received. These benefits may not be used with any other discount plan or program. Providers are subject to change without notice, and programs may vary in some states and are not available in all states.

This program is not a substitute for health insurance, and you should not drop any existing health insurance in reliance on it. NOSA is a marketer of this program; it is not the discount plan organization and does not operate the program, contract with providers, or set provider prices.
